Area nursing home facilities cited for alleged violations

The Illinois Department of Public Health’s 2nd Quarterly Report showed the following 13 facilities cited for violations, with fines totaling $143,600.

The Illinois Department of Public Health has initiated action, as indicated, against the following facilities which have been determined to be in violation of the Nursing Home Care Act, or has recommended decertification to the Director of the Department of Healthcare and Family Service, or the Secretary of the U.S. Department of Health and Human Services for violations in relation to patient care, pursuant to Titles XVIII and XIX of the Federal Social Security Act.

  • Type C violation. The fine was doubled due to the violation of the sections of the Code with a high-risk designation. A fine of $500.

Alleged violation types

TYPE A VIOLATION cited by the IDPH, according to the state, is an alleged violation of the Nursing Home Care Act or its rules that creates a substantial risk of serious harm or death to a resident. It can also be a violation that has already resulted in physical or mental harm to a resident.

TYPE B VIOLATION is an alleged violation of the Illinois Nursing Home Care Act or its rules that increases the likelihood of a nursing home resident suffering minor physical or mental harm, IDPH says.

TYPE C VIOLATION is alleged violation of the Nursing Home Care Act or its rules that could cause serious injury or death to residents, employees, or the public. The Illinois Department of Public Health (IDPH) may take immediate action, such as closure, to address Type C violations. The IDPH may also impose fines of $100 per violation, per day.

    My Sister was sent to rehab @ The Loft in East Peoria..This was such a horrible experience for her and myself as well. She wasn’t taken care of in any professional manner! It smelled like BM & strong urine. One night she was woken by a man with his hands under her blanket in her private area. She asked him what he was doing? He replied checking to see if you’re wet? She told him to leave her room, He bent her wrist back on her bad arm & asked if it hurt! She began screaming. The next day she informed the nurse on duty & she told her, There was no male working the night before! There is a collage not far from the Loft & it made me wonder?

    i cant believe odin illinois health care hasnt been shut down. my mom was there for 2 years and was treated horrible. it was a constant fight with them. trying to use dirty machines, leaving people in urine, screaming at them. i seen all of this and nothing gets done about it.

    If your employees can’t do there job to keep the residents safe from falling or even something that’s causing death I think it’s time to get new workers . I worked myself into the dirt to make sure someone else loved ones were taking care of . I loved them like they were my own grandparents. How would you feel if your loved one was in a facility getting ratings and fines like this from pubic health? And yes I know there’s going to be some Karen’s somewhere out there saying something in the lines of , well if they don’t like they can take care of the family member at home. I been in healthcare for over 19yrs I heard it all!! But until you try and take care of your own loved one that has dementia or some other horrible cribbling disease it isn’t easy one bitc. And yes we do have the one that just wanna drop them off and only show up when it’s an emergency when they are called . But come on Healthcare Workers We Are The Heart Of America!!!!! DO BETTER
